Our Trusted Assessor team works across all hospitals in Bridgend, Merthyr Tydfil and Rhondda Cynon Taf, providing patients with the practical and emotional support they need to leave hospital safely, quickly, and with confidence.

Returning home after a hospital stay can often feel overwhelming, especially for those who have been unwell or have limited support. Our team ensures that patients and their families feel reassured, informed, and fully supported throughout the discharge process.


We work closely with patients, their families, and in partnership with health, social care, and community organisations to provide a coordinated approach to hospital discharge. By working collaboratively, we can help people return home safely, reduce the risk of re-admission, and support independence and confidence.


How Our Trusted Assessors Help

Our Trusted Assessors provide ongoing, free support tailored to each individual’s needs. This includes practical help to readjust to life at home, reassurance and emotional support, and guidance to regain confidence and independence. The team can also offer advice and liaison with other agencies, support for carers and family members, and immediate provisions such as food until regular shopping arrangements can be made.


Additional support includes guidance with welfare benefit claims and essential home safety checks to reduce the risk of slips, trips, and falls. This may involve clearing out-of-date food, checking that heating, TV, phone, and lifeline services are working correctly, and identifying any other potential hazards in the home.


Making a Real Difference

The Trusted Assessor team has helped countless people transition safely from hospital to home. For example, PB was medically fit for discharge but had no home, clothes, or food. Our team stepped in to provide essentials and liaise with housing services, ensuring a safe and timely discharge. In another case, AN, who had spent a prolonged period in hospital following multiple strokes, faced challenges settling back at home. Our Hospital Discharge Team provided both practical and emotional support, helping him reconnect with familiar services and regain independence.


Every day, our Trusted Assessors make a real difference in people’s lives, helping patients leave hospital with confidence, supporting families, and working alongside a network of local services to ensure a smooth transition home.


We are available Monday to Friday, 8:30am–4:00pm, on-site in hospitals across Bridgend, Merthyr Tydfil, and Rhondda Cynon Taf.
