Our work is rooted in local communities, providing practical, emotional, and trusted support to help people live well, stay independent, and feel connected.

We deliver a range of services across Bridgend, Merthyr Tydfil, and Rhondda Cynon Taf, working alongside individuals, families, and partner organisations to make sure the right help is available at the right time.


Hospital Discharge Support

Our team works across all hospitals in Bridgend, Merthyr Tydfil, and Rhondda Cynon Taf, supporting patients to leave hospital safely, quickly, and with confidence.

We provide practical and emotional support at a key moment of transition, working closely with individuals, their families, and partners in health, social care, and the wider community to ensure the right support is in place at home.


Dementia Support

This service is dedicated to supporting people living with dementia and their carers through every stage of their journey, from diagnosis through to end of life.

Each person is supported by a named Memory and Wellbeing Guide, who provides ongoing advice, emotional support, and signposting to appropriate services. We also work closely with people with lived experience to ensure the service is shaped by those who use it.


Digi Cafes

Digital skills play an important role in helping older people stay connected, independent, and informed. Our Digi Cafés provide friendly, informal support to help people build confidence with technology. From video calls and social media to online banking, shopping, and accessing health and government services, we help people gain the skills they need for everyday life, reducing isolation and increasing independence.


Advice Pop-Ups

The Info Street is our pop-up advice service for people aged 50+, bringing free, trusted information and support directly into local communities.

Our Information & Advice Team and EngAGE Team work together to deliver sessions in familiar local venues across Rhondda Cynon Taf, Merthyr Tydfil, and Bridgend. Whether it’s a quick question or more detailed support, we are here to help. Our aim is simple: to help people save money, maximise their income, and build the confidence and skills needed to stay informed, independent, and connected.
