Groundbreaking pilot launches at Prince Charles Hospital

Age Connects Morgannwg have launched a groundbreaking hospital discharge pilot project, called My Passport Home, at the Prince Charles Hospital in Merthyr.

On Monday, 16th December 2024, Age Connects Morgannwg unveiled an innovative pilot project, My Passport Home, designed to enhance the hospital discharge process for older patients at Prince Charles Hospital in Merthyr. This initiative aims to provide patients with comprehensive information and support, ensuring a smoother transition back home.

 

What is  My Passport Home?

From Monday, 16th December, to 31st January, two of our dedicated Trusted Assessors, Kim and Ceri, are piloting the My Passport Home project with patients aged 50 and over, as well as their families. This passport is a personalised tool that supports hospital discharge by ensuring patients have everything they need for a safe and timely return home. It includes:


  • Contact details for the Trusted Assessor assigned to their care.
  • A checklist of essentials the patient will need upon returning home.
  • Comprehensive details of their care package.


The goal of the pilot is to equip patients with clear, accessible information and connect them with the right resources to make their journey home as stress-free as possible.

 

The Trusted Assessor Project

Our Trusted Assessors are at the heart of this initiative, offering expert guidance and support to patients navigating the hospital discharge process. Based at Prince Charles, Royal Glamorgan, and Princess of Wales hospitals, they work closely with patients to:


  • Facilitate a safe and well-organised discharge.
  • Address concerns about returning home.
  • Provide tailored advice and connect patients to necessary services.


Patients can self-refer to our Trusted Assessors or ask a member of hospital staff to contact the team on their behalf. Our Trusted Assessors are frequently on the wards and are always happy to chat and provide assistance to patients.

A Collaborative Effort

My Passport Home is a joint initiative from Cwm Taf Morgannwg University Health Board and Age Connects Morgannwg and is funded by the Regional Integration Fund. This collaborative approach ensures that older patients receive the support they need at a crucial stage of their recovery.
