About Us

Age Connects Morgannwg is the leading charity working with older people living in Rhondda Cynon Taf, Bridgend and Merthyr Tydfil (the Cwm Taf Morgannwg Health Board footprint). We have a strong history with over 45 years of supporting older people and their families through some of the most difficult times in their life. We also have a proven track record of listening to older people, learning what’s important to them and caring about the way they are treated, perceived, and portrayed.   

About the Job

Are you looking for a new opportunity to be at the start of designing a new programme of work, do you have a passion to deliver services for people with dementia that makes a real difference to their lives.


We are recruiting an experienced Programme Manager who will have significant knowledge and experience in working in the dementia field. This new role offers the scope to deliver a newly initiated programme funded by Cwm Taf Morgannwg Regional Integration Fund and as set out in the “All Wales Dementia Care Pathway of Standards” (the standards) to provide a named contact (connector) to offer support, advice and signposting to people with dementia and their carers throughout their journey from diagnosis to end of life. 


This directly links to the priorities of the Cwm Taf Morgannwg Regional Partnership Board which brings people together to improve services and support in Cwm Taf Morgannwg. This role was coproduced by people with lived experience across the Cwm Taf Morgannwg region and is an integral part of our regional approach to the delivery of the standards.


Your duties and responsibilities will be informed by what matters to people living with dementia in the Cwm Taf Morgannwg region. This role will both manage the programme and hold a small caseload. 


You will also be the type of person who enjoys identifying new opportunities and challenging the status quo, ensuring that ACM’s wider Dementia Matters programme responds to what people with dementia tell us and contributing to the delivery of the Regional Partnership Board’s area plan priorities as identified by people living in our region.


The role will manage a small team of Dementia Connectors and will be delivered throughout the Cwm Taf Morgannwg region which includes the local authority areas of Bridgend, Merthyr and Rhondda Cynon Taf.
